NRVLC High School Players and Parents,
Please read the following information regarding the Driver and Passenger Permission forms very carefully.
-ANY players planning to drive themselves to an away game must turn in a completed permission form, signed by their parent, to their coach.
-ANY player planning to drive a teammate(s) must have their parents signed permission to do so on the completed permission form their coach receives. Each parent-approved passenger must be listed by name on the signed Driver Permission form.
-ANY player planning to ride with a teammate who is driving to the game must turn in a completed permission form signed by their parent to their coach. The parent-approved driver must be indicated by name on the signed Passenger Permission form.
-The forms will be available at www.nrvlc.org
-A new form must be completed for EACH away game.
-ANY player who does not turn in the proper form will NOT be allowed to drive herself or ride with a teammate. There are NO SUBSTITUTES for the form - phone calls from parents, handwritten notes, etc. will not be accepted.

NRVLC AWAY GAME DRIVER PERMISSION FORM
I, __________________, as the parent/legal guardian of ______________________ give my permission for him/her to drive himself/herself to and from the lacrosse game their NRVLC team will be playing on (date)_____________ ____, 2010 at (location)____________________________________. I have considered the distance to be traveled, time of travel, weather forecasts, road conditions, and driving experience of my child in making this decision.
Furthermore, My child has my permission to allow the following teammates as passengers in his/her vehicle to and from the above-mentioned away game:

Please note that each passenger must also turn in a completed Passenger Permission Form signed by his or her parent, and listing the parent-approved driver by name, to their coach. If this form is not submitted a passenger will NOT be allowed to ride with any player driver regardless of whether or not the driver has permission to transport them.
I HAVE READ THE ABOVE PROVISIONS AND UNDERSTAND THAT BY SIGNING THIS FORM I HAVE GIVEN MY CHILD PERMISSION TO DRIVE HIMSELF/HERSELF AND ANY ADDITIONAL PASSENGERS LISTED ABOVE BY NAME TO THE PRIOR MENTIONED AWAY LACROSSE GAME.

NRVLC AWAY GAME PASSENGER PERMISSION FORM
I, __________________, as the parent/legal guardian of ______________________ give my permission for him/her to ride as a passenger of his/her teammate (list player driver by name) _________________________ to and from the lacrosse game their NRVLC team will be playing on (date)_____________ ____, 2010 at (location)____________________________________. I under stand that this teammate will be the driver of the vehicle my child is riding in and I have considered the distance to be traveled, time of travel, weather forecasts, road conditions, and driving experience of this driver in making this decision. I am also aware that this driver may be transporting teammates, in addition to my child, to and from this game.
Please note that each passenger must also have their name listed and initialed by the player-drivers parent on a completed driver permission form signed by the driver’s parent and turned in to their coach. If this form is not submitted or their name is not listed and initialed by the driver’s parent, they will NOT be allowed to ride with any player-driver regardless of whether or not they have their own parents permission to do so.
